NCN Health is a registered provider of the Australian Government's Support at Home Program, which offers a wide range of coordinated care and support services for older people. Support at Home came into effect on November 1, 2025 and was previously known as the Home Care Package Program.
Support at Home ensures improved access to services, equipment and home modifications to assist older people to remain living in their homes safely and independently.
When you (the participant) have been allocated Support at Home funding, you will be required to choose a provider to manage your funds and services.
NCN Health staff, known as Care Partners, will work with you (the participant) to coordinate your services and maintain your budget.
Services are tailored to meet your assessed needs. Some examples of services you may use are; nursing care, allied health therapy, personal care, home maintenance, domestic assistance, meals and more.
