Our District Nursing Service provides clinical nursing support in the home. We offer a range of services that can include:
- Wound management
- Medication monitoring, management and education
- Diabetes education, support and management
- Stomal and catheter care
- General health monitoring and education if you are or have been unwell
- Palliative care support
Where is it available?
District Nursing Services are available across Nathalia, Cobram and Numurkah communities and are usually provided as a visit to your home.
You may prefer to be seen by a District Nurse at one of our Community Health facilities, and this can be arranged.
What does it cost?
You may need to pay a small cost towards any items such as bandages and wound dressing products.
How can I access it?
Most people get a referral from their doctor. You can refer yourself or be referred by another health professional, hospital, family, carer or advocate provided you give your consent for this.
