The Transitional Care Program (TCP) provides goal orientated, time limited and therapy focused care, to help older people transition from hospital to home confidently and independently. The TCP program involves moving from hospital to one of our TCP rooms at NCN Health where patients prepare for the transition back to independent living. The program is conducted in partnership with Goulburn Valley Health.
What does it cost?
There is no cost to the patient. Patients are accessed by the TCP Team and must meet certain criteria to be eligible for the program.
